USSSA Roster Manager Affidavit

READ THE FOLLOWING STATEMENT

It is the managers responsibility to insure that all players added to the roster are legal and eligible according to the Official USSSA {{sportName}} National By-laws. Approval of your roster and additions only indicates that you are within the number allowed on your roster and/or allowed to add after your roster becomes frozen. It does not approve the legality or eligibility. If players are found to be ineligible or illegal, you and your team may incur penalties.

In consideration of being permitted to participate in the U.S.S.S.A., I hereby agree for myself, my successor, heirs and assigns, Release and forever discharge the (U.S.S.S.A.), their employees, officers and directors from all claims, actions or judgments I may have or claim to have against U.S.S.S.A. - for all personal injuries, including death, and injuries to property, real or personal, caused by or arising out of my participation in the U.S.S.S.A.,- either Leagues or Tournaments. I further agree for myself, successor, heirs and assigns to indemnify and hold U.S.S.S.A. harmless from all claims and suits for personal injuries, including death, damages to property caused by my act of commission arising out of our participation in the U.S.S.S.A., and from all judgments recovered and from all expenses incurred in defending said claims or suits.

I further agree that my photographs, pictures, slides or movies taken or made by U.S.S.S.A., their employees, officers and directors, in connection with my participation in the U.S.S.S.A. - either Leagues or Tournaments, or any reproduction of the same, as well as my name, may in any manner be used by U.S.S.S.A., or by any person, corporation or association authorized by U.S.S.S.A.

I am in good health and have no physical condition that would prevent me from participating in U.S.S.S.A. events.

U.S.S.S.A. Requirements: Adult Team Members May Be Asked To Provide Picture I.D. Upon Request.

Team Managers Affidavit: I am the manager of the submitted team. and guarantee all of the information supplied above is correct to the best of my knowledge and that all of the players signed in the above, by providing your their valid date of birth, that they are eligible to compete with my team in the championship play of the U.S.S.S.A. I also guarantee that if my team is a church team, all members are members in good standing of the church that we represent. Team roster must be submitted to the Regional, Director upon qualifying for State or National Championship play.

Note: If you have not entered the correct date of birth for all players you may lose a player eligibility protest. If you need to update the birthdate of any player please email contact your state director
